The consultation request form collects only your name, email, phone number, state, the age of the person to be evaluated, the main area of concern, and how you heard about us. Please do not include detailed clinical information in the form.
How we use it
We use this information only to respond to your enquiry and to schedule a consultation. We do not sell or rent your information.
Clinical information
Clinical information, records, and payments are handled inside a separate, secure, HIPAA-compliant client portal after your consultation, and are covered by our Notice of Privacy Practices.
